4. Telehealth Services & Consent (AB 809)
Telehealth services involve the use of electronic communications to provide healthcare services remotely. By scheduling or participating in telehealth services, you acknowledge and consent to the use of telehealth technology.
You understand that:

Telehealth may involve risks such as technical failures, interruptions, or unauthorized access despite reasonable security safeguards

Telehealth may not be appropriate for all clinical situations

You may withdraw consent to telehealth services at any time by notifying the practice
Additional telehealth consent is obtained during the intake process prior to the initiation of clinical services.
5. Telehealth & Jurisdiction
Telehealth services are available only to patients who are physically located within the State of California at the time of the appointment. By scheduling services, you attest that you are physically present in California.
6. Financial Policy & Good Faith Estimate

Cancellations: Appointments cancelled with less than 24 hours' notice are subject to a $150 cancellation fee, which is not covered by insurance.

No Surprises Act: Uninsured or self-pay patients have the right to receive a Good Faith Estimate of expected charges upon request prior to scheduling.
7. Automated Communications (AI Disclosure)
Portions of administrative communications, such as appointment reminders or intake summaries, may be generated using automated systems or artificial intelligence. All clinical care decisions are made by a licensed human provider.
8. Governing Law
These Terms of Use are governed by and construed in accordance with the laws of the State of California. Any legal action arising from the use of this website shall be brought exclusively in a court of competent jurisdiction within California.
